Guidelines

Patients must:

1.

Provide proof of income
Acceptable documents: 1040 form OR three recent paycheck stubs, OR a notarized document from someone willing to be responsible for patient’s bill

2.

Must meet income criteria

Patient cannot have any type of insurance

3.

Provide proof of address, ID, and number of individuals in the patient household
Acceptable documents: utility bill OR gas bill, OR mail that has patient’s name connected with the address they are using

4.

Provide Social Security cards OR tax ID for each member being counted in the household

FAQs

InterFaith is not a walk-in clinic. Patients of InterFaith must qualify by completing a patient application and returning it to InterFaith, or by calling 865-546-7330.

InterFaith does not accept insurance or TennCare and does not see patients that have insurance or TennCare.

InterFaith does not dispense, or prescribe narcotics and does not offer MAT medications.

InterFaith is not a free medical center. Patients pay an adjusted fee for their appointments and services based on their household income.

InterFaith is currently accepting new medical patients. Individuals seeking to become patients at InterFaith may come by the center to complete an application packet any time between the hours of 8:30 -11 am or 2-4 pm. Please note that, due to patient volumes, new patient applications may only be completed during these designated times.
